Credit Card Authorisation Form

If you are paying for a guest with a 3rd party credit card, fill out and send us this form to authorise us to charge your personal or company credit card.  Please note: For guests who do not have an "All Charges" authorisation, we will ask for a credit card, EFTPOS or cash deposit from the guest to guarantee for incidentals at check in.
